-Birth Certificate or verification of birth -Immunization Records for your child
GSRP Enrollment Information Children must be age 4 on or before September 1st. Required Documents necessary for enrollment: -Great Start Readiness Preschool Application
-Birth Certificate or verification of birth -Immunization Records for your child -Copy front and back of State ID -Parent/Guardian 2024 1040 taxes, W2s, or 3 pay stubs per working adult
